The visiting registered nurse plans, organizes and directs patient care. RN is experienced in nursing, with emphasis on community health.The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and their families within their homes/ places or residence and communities. Completes an initial assessment of patient and family to determine home health agency needs.Provides a complete physical assessment and history of current and previous illness(es). Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.Records pain/symptom management changes/outcomes as appropriate.


  1. Graduate of an accredited school of nursing
  2. Current licensure in the State of Pennsylvania, and a valid driver's license
  3. Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills
  4. Able to lift, position and /or transfer patients.
  5. Able to lift supplies and equipment.
  6. Considerable reaching, stooping, bending, kneeling and/or crouching.
  7. Visual acuity and hearing to perform required nursing skills
  8. Agency may review new graduates application, and it is to the agency’s discretion if we the agency, choose to accept the application and fulfill proper training with shadowing of

other nurses.
